i guess he just lost either a (or several) decon items or the (heroic) catalyst itself due to some sort of bug - petitioned and the gm gave him the wrong sort of catalyst back - (wich was fixed shortly after...)

That is exactly what happened Pheelon. The GM gave him the wrong item by a mistake.
