According to Brad the portals will be at sea, not in harbors or anything silly like that. You'll have to sail from your port of origin to a portal, and then from the portal you arrive at to your destination port. There won't be portals going to/from island dungeons. The portals are being incorporated into the lore, and he mentioned using portals with "random" destinations (seemed like he was hinting at high-end content there.)
Minor correction: the portals are indeed out in the ocean, so you have to sail to them like you said, but then you also appear in the ocean, not at the docks of the next continent, so you still have to sail some more. What they do is allow you to skip quite a few of the ocean chunks if you wish, but they are NOT portals that are right there when you leave Qalia and pop you right there at the shores of Thestra when you exit them.
Travel time on the ocean, just like the land, is still important and there - these teleports just eliminate some of the long ocean journey that would have been necessary given the size of the ocean and the fact that there's no way we can make all of the ocean chunks interesting and fun to travel through like land chunks.
